NKTI medicine assistance documents checklist: ano ang dadalhin bago pumila
Current checklist built from repeated August–September 2026 first-hand NKTI medicine claims. It separates documents patients repeatedly used from items that appear only in some cases.

Repeated in successful recent claims
Original prescription and a photocopy appear repeatedly across recent patient reports. Valid ID and GL/reference details are also common.
One patient specifically prepared two photocopies each of ID, receipt and laboratory request/prescription before the hospital assistance step.
Useful to bring even when not always requested
Extra photocopies, latest labs, laboratory or imaging requests, quotation and prior medical records are repeatedly useful because the exact counter requirements can change.
The point of this list is not to claim that every document is mandatory every day. It is to reduce the chance of being sent out to photocopy or retrieve a missing paper.
If the request includes labs or imaging
Recent KidneyTalksPH patient-facing reports warn that requests should be attached before assistance crediting when you want them included; otherwise the transaction may be classified for pharmacy only.
Show all medicine, laboratory and imaging requests together and ask what is actually being credited.
If a representative will claim
Bring patient ID copy, representative ID, prescription, assistance documents and a signed authorization letter at minimum while confirming the exact rule.
Recent comments conflict on whether some cases require SPA, so this remains a verify-before-travel item.
Post-transplant Z Benefit is a different document set
A September 18–19 first-hand Post-KT claim reported Patient Passport copy, Laboratory Voucher, Tacrolimus voucher, Annex K1 and Annex E.1 checklist. This is not the same document set as a GL medicine claim.
