Ang HOPE o Human Organ Preservation Effort ay bahagi ng NKTI organ-donation and transplantation program. Para sa patient na gustong maging candidate para sa kidney mula sa deceased donor, kailangan munang dumaan sa transplant evaluation at waiting-list enlistment process.
Sa national framework for deceased-donor kidney allocation, ang candidate ay ine-enlist muna sa transplant center of choice. Kapag kumpleto at accepted ang requirements, ipinapasa ng transplant center ang required information para sa national waiting list through PhilNOS. Hindi ibig sabihin na dahil nakapag-submit ka na ay automatic na active ka na o may kidney na agad.
Ang matching ay hindi simpleng “unang nag-enlist, unang tatawagan.” Blood group, tissue compatibility, antibodies/PRA, clinical suitability and allocation rules all matter when a deceased donor becomes available.
Step-by-step
Mula referral hanggang waiting-list maintenance
Start with the transplant/nephrology team
Kailangan munang ma-assess na transplant candidate ka. Humingi ng referral at updated medical abstract. Para sa service/charity pathway, maaaring dumaan muna sa Medical Social Service para sa referral and financial assessment.
Get the current HOPE checklist

Complete immunology and screening tests

Under the Philippine deceased-donor allocation framework, the transplant center forwards eligible candidate information for inclusion in the national transplant waiting-list system managed through PhilNOS.
Keep your status active and file updated
Enlistment is not the end of the process. Repeat serum, PRA, viral screening or clearances when due, keep your phone number updated, and immediately report major health changes to the transplant team.

Additional tests or clearances you may be asked to complete
These items appear in more recent patient guidance but are not all listed the same way in the older public HOPE checklist. Treat them as current-practice clues that still need confirmation from HOPE.
- •Chest X-ray, ECG at 2D Echo may be requested as part of current pre-transplant clearance workup.
- •Pulmonary clearance may be required if clinically indicated.
- •For women, Pap smear/gynecologic clearance may be requested depending on current transplant-center requirements.
- •A nephrology clearance or updated assessment may be required before or during final enlistment review.
- •Dental clearance validity and whether it is mandatory should be confirmed with HOPE because public checklists and newer patient instructions may differ.
Bakit mahalaga ang PRA?
PRA helps the transplant team understand whether you have antibodies that may react against potential donor tissue. A higher degree of sensitization can make matching more difficult, so some candidates need more detailed antibody testing.
The older HOPE checklist states that a positive PRA screening result or a previous transplant may trigger a PRA single-bead antigen/PRA-specific test. Your transplant team should decide which test is needed and when it must be repeated.
Serum storage: bakit hindi natatapos sa unang sample?
Stored serum can be used for donor-recipient compatibility work when a potential deceased donor becomes available. Candidates may be asked to submit fresh serum at intervals set by the transplant program. Keep a personal calendar of your next serum date and follow the exact schedule HOPE gives you. A missed or outdated sample can complicate the ability to perform timely compatibility testing.
After enlistment

- ✓Keep your registered mobile number active and answer calls from the transplant center.
- ✓Track PRA, serum, viral screening and clearance expiry dates.
- ✓Keep digital and paper copies of results and receipts.
- ✓Tell the transplant team about hospitalization, infection, surgery, blood transfusion, pregnancy or other major medical changes because these may affect testing or eligibility.
- ✓Continue dialysis and prescribed medicines unless your own clinical team tells you otherwise.
- ✓Have a transport/contact plan in case HOPE calls about a possible deceased donor.
Kapag may possible donor call
A call from HOPE or the transplant team means there may be a potential donor opportunity. Hindi pa ito guarantee na ikaw ang makakatanggap ng kidney. Final donor assessment, compatibility testing/crossmatch and your current medical condition still need to be checked.
Follow the exact instructions you receive about when to come to NKTI, whether to eat or drink, dialysis timing, medicines and documents. Do not stop medicines or skip dialysis just because you received a possible-donor call unless the transplant team specifically tells you to do so.
