NCR online, provinces through PCSO branches or hospital-assisted channels
Sa 2025 PCSO Citizen’s Charter, ang NCR Medical Assistance Program (MAP) ay may sariling online application process. Ang applicant ay pupunta sa PCSO website, pipili ng E-Services at Online MAP, maglalagay ng patient information at mag-u-upload ng scanned documentary requirements. Ang current PCSO Online MAP portal ay active pa rin at gumagamit ng account/login at OTP verification.
Para sa branch offices outside the NCR online route, ang Citizen’s Charter ay may hiwalay na MAP process sa PCSO Branch Offices nationwide. Ang listed branch service hours ay Monday to Friday, 8:00 AM–5:00 PM. Kung nasa hospital na may PCSO/Malasakit processing, puwede ring i-coordinate muna sa Medical Social Service, Billing o Malasakit Center para malaman kung saan dapat dalhin ang case.
PCSO assistance is generally implemented through a Claim Slip or Guarantee Letter (GL) addressed to a Partner Health Facility (PHF), gaya ng hospital, dialysis center, diagnostic facility o participating supplier. Hindi ito dapat i-present na automatic cash assistance.
NCR: paano ang official online MAP process?
- 1Pumunta sa official PCSO website at piliin ang E-Services → Online MAP Application.
- 2Create an account kung first time, then complete the registration and validation steps shown by the portal.
- 3Sa login, gamitin ang patient/applicant details and complete the OTP verification required by the Online MAP system.
- 4Start the application, choose the Partner Health Facility (PHF) and nature of request, then complete the queuing/application step shown by the portal.
- 5Upload the required documents for the exact assistance category. Keep clear PDF scans ready before starting.
- 6Submit the case and monitor the application status or PCSO notification. If approved, follow the portal instructions for the Claim Slip/Guarantee Letter and present the required originals to the PHF.
General documents: ihanda muna ang basics
The exact checklist depends on the type of request. Across the current PCSO MAP procedures, the recurring basics are patient identification plus a current medical document signed by the attending physician, then the category-specific bill, prescription, request or quotation.
Patient ID
Government-issued ID or another ID accepted by the PCSO checklist for the applicant category.
Medical document
Medical Abstract for inpatient cases or Medical Certificate/Clinical Abstract for outpatient or treatment requests, with the attending physician’s identifying details and license number.
MAP form
PCSO MAP/IMAP application form when required by the route being used.
Representative documents
If a representative is filing, confirm the additional ID or authorization requirements with the processing office or hospital desk.
Hemodialysis: PCSO’s current official checklist
For hemodialysis under the PCSO Main Office/NCR checklist, the 2025 Citizen’s Charter specifically lists:
- ✓Official quotation from the dialysis center or hospital.
- ✓For PhilHealth members, certification on the number of benefits availed.
- ✓The applicable general medical document and patient ID required by the MAP route.
For branch-office dialysis applications, the Citizen’s Charter also lists an acceptance letter from the dialysis center/hospital confirming acceptance of a PCSO Guarantee Letter, together with the branch-specific supporting medical documents. Always use the checklist for the office actually handling your case.
Other common request types
Hospital confinement
Final/latest Statement of Account or hospital bill with applicable deductions; if discharged, the Citizen’s Charter may require a valid hospital promissory note/certification showing the remaining balance.
Chemotherapy
Prescription, treatment protocol and the quotations required by the current checklist, plus the general medical documents.
Laboratory / diagnostic
Official quotation from the diagnostic center/hospital and the attending physician’s laboratory or diagnostic request.
Implants / medical devices
The current Main Office checklist calls for three official quotations from different suppliers plus the physician’s specification/prescription for the device.

The current official material we verified clearly supports the NCR online route, the branch-office route, GL/Claim Slip assistance, category-specific requirements and a within-24-hours NCR processing target.
Some widely shared 2026 applicant instructions—such as a fixed daily 8:00 AM online quota, an NCR email fallback, a new rule allowing GLs to cover doctors’ professional fees, and different one-day/two-day release times by request type—were not stated in the PCSO Citizen’s Charter pages we verified. We are therefore not presenting those points as confirmed PCSO policy here until an official PCSO circular, advisory or current portal notice can be linked directly.
This also avoids confusing PCSO MAP with other government medical-assistance programs. A change announced by another agency does not automatically change PCSO’s GL procedure.
Practical preparation before you apply
- ✓Ask the hospital Social Service/Billing/Malasakit desk whether the facility accepts PCSO Guarantee Letters and which PCSO route they use.
- ✓Prepare clean digital copies of your ID, medical abstract/certificate, bill or quotation and the request/prescription that matches the assistance category.
- ✓Do not rely on screenshots of old checklists. Compare your documents with the current PCSO Citizen’s Charter or instructions shown in the Online MAP portal.
- ✓Keep your application reference, submission date and copies of every document you uploaded or submitted.
- ✓Do not purchase an expensive medicine, implant or procedure expecting reimbursement unless the applicable PCSO/PHF process has been confirmed first.
