After dialysis, caregivers often see symptoms the clinic may not witness: extreme fatigue, dizziness, cramps, nausea or weakness after reaching home.

A simple symptom record can turn vague complaints into useful information for the dialysis team.

Record the pattern

Note when symptoms begin, how long they last, whether they happen every session and whether the patient can walk, eat and function normally afterward.

Also note unusual events during that dialysis session if the patient remembers them.

Help with safety, not treatment changes

If the patient is lightheaded, help them sit or lie safely and follow individualized instructions from the centre.

Do not independently change dry weight, fluid allowance, blood-pressure medicines or dialysis time.
