Ano ang ibig sabihin ng eGFR ranges?
Sa CKD classification, ang GFR categories ay: G1 = 90 or higher, G2 = 60-89, G3a = 45-59, G3b = 30-44, G4 = 15-29, at G5 = below 15 mL/min/1.73 m².
Pero hindi ibig sabihin na lahat ng may eGFR 60-89 ay may CKD. Sa G1 at G2, kailangan may ibang persistent marker ng kidney damage gaya ng albuminuria/protein sa ihi, abnormal urine sediment, structural abnormality sa imaging, o ibang documented kidney abnormality para ma-classify bilang CKD.

Normal ba ang eGFR na 82?

Age matters because average eGFR tends to decline with age even in people without known kidney disease. Kaya hindi tama na i-label agad ang 82 bilang 'mahina na ang kidney' nang walang context.

Bakit kailangan ng urine ACR kahit okay ang eGFR?
Ang eGFR ay nagsasabi tungkol sa filtration; ang urine albumin-to-creatinine ratio o UACR ay naghahanap ng albumin leakage na marker ng kidney damage. Magkaibang impormasyon ang dalawang tests, kaya parehong mahalaga sa CKD assessment.
Puwedeng normal o above 60 ang eGFR pero may clinically important albuminuria. Sa kabilang banda, puwedeng mildly reduced ang eGFR pero walang persistent albuminuria o ibang kidney-damage marker.

Ano ang UACR categories?
Common CKD albuminuria categories are A1 below 30 mg/g, A2 from 30 to 300 mg/g, and A3 above 300 mg/g. Abnormal urine albumin often needs confirmation because exercise, fever, infection and other temporary factors can affect the result.
A persistent UACR of 30 mg/g or higher can be evidence of kidney damage even when eGFR is 60 or higher.
Bakit puwedeng mag-iba ang eGFR kahit walang permanent kidney damage?
Because eGFR is calculated largely from creatinine, anything that changes creatinine or kidney perfusion can affect the estimate. Examples include dehydration, acute infection or illness, hospitalization, medicines that affect kidney hemodynamics, major changes in muscle mass, intense exercise, and some dietary or supplement factors.
A change does not automatically mean permanent damage. What matters is whether the change persists, whether the creatinine is stable, whether urine tests show kidney damage, and what caused the change.
Kailan maaaring gumamit ng cystatin C?
Cystatin C is another filtration marker that can be combined with creatinine to estimate GFR. NIDDK notes that the combined creatinine-cystatin C equation is generally more accurate than creatinine alone.
It can be especially useful when creatinine may be less reliable—such as unusual muscle mass—or when the eGFR is near an important clinical decision threshold. Availability and cost vary, so the treating clinician decides when it adds value.
eGFR at diet: kailangan bang may bawal agad?
Hindi dapat gumawa ng automatic renal diet restriction base sa eGFR number lang. Sodium, potassium, phosphorus, protein at fluid advice depend on the diagnosis, laboratory results, blood pressure, medicines, urine output, CKD stage and whether the person is on dialysis.
Halimbawa, ang eGFR na 82 na walang hyperkalemia, significant albuminuria o ibang specific indication does not by itself justify a highly restrictive renal diet. Unnecessary restriction can make nutrition harder.
Kung may AKI, dehydration, diabetes, hypertension or another condition, the appropriate diet may be driven by that condition and the recovery plan rather than the eGFR number alone.

Useful follow-up usually includes the date, serum creatinine, eGFR, UACR or urinalysis result, blood pressure, major illnesses, medication changes and whether the patient was dehydrated or hospitalized around the time of testing.
This timeline helps distinguish a transient event from a persistent pattern. If the result followed AKI, ask the nephrologist when kidney function is considered stable enough for chronic staging.

- ✓Record major illness, dehydration and medication changes beside the lab date.
- ✓Ask whether repeat urine testing or imaging is needed based on your case.
Kailan mas kailangan ng prompt medical review?
A rapidly rising creatinine, rapidly falling eGFR, very little or no urine, severe vomiting or dehydration, worsening swelling, shortness of breath, confusion, chest pain, severe weakness or other acute illness needs timely clinical assessment.
The urgency depends on the whole situation, not just the eGFR number. Follow the instructions of the treating doctor or nephrologist, especially after a recent AKI or hospitalization.
Ano ang magandang itanong sa doctor o nephrologist?
Ask whether the current eGFR is stable, recovering or declining; whether you meet CKD criteria; whether UACR or urinalysis shows kidney damage; and when labs should be repeated.
If the eGFR is near a medication-dosing, imaging, surgery or transplant decision threshold, ask whether a more precise estimate such as creatinine plus cystatin C would be useful.
